Position Overview:
As a Family Medicine Nurse Practitioner (NP) or Physician Assistant (PA), you will play a vital role in providing primary healthcare services to individuals and families across the lifespan. You will collaborate with a team of dedicated healthcare professionals to deliver holistic and patient-centered care, emphasizing preventive care, health promotion, and disease management.
Responsibilities:

Patient Assessment: Conduct comprehensive health assessments, including medical history, physical examinations, and diagnostic tests, to formulate accurate diagnoses and treatment plans.

Diagnosis and Treatment: Develop and implement evidence-based treatment plans, prescribe medications, order and interpret diagnostic tests, and provide therapeutic interventions to manage acute and chronic health conditions.

Health Education: Educate patients and their families about health maintenance, disease prevention, and treatment options, ensuring that they understand their healthcare plans and are empowered to make informed decisions.

Chronic Disease Management: Manage and monitor patients with chronic diseases such as diabetes, hypertension, and heart disease, offering continuous support and adjustments to treatment plans as needed.

Preventive Care: Administer vaccinations, perform routine screenings, and counsel patients on lifestyle modifications to promote optimal health and prevent illness.

Collaboration: Collaborate with other healthcare providers, including physicians, nurses, and specialists, to ensure coordinated care and referrals as necessary.
Qualifications:
- Master’s degree in Nursing (NP) or Physician Assistant Studies (PA-C).
- Current state licensure as a Nurse Practitioner (NP) or Physician Assistant (PA).
- Prior experience in family medicine or primary care is preferred.
